Individuals may be nominated on the website. If chosen for consideration, an essay explaining why the nominee should receive help from the Monte Foundation will be submitted along with a letter from their physician verifying the nominee’s condition and disability.

NOMINATION PROCESS

Nominee submissions will be reviewed by the Board of Directors, or a Selection Committee, and one or more recipient will receive assistance, free of charge, in the form of the following:
• Installation of a wheelchair ramp
• Reconstruction to home entrances to widen doorways allowing for wheelchair accessibility.
• Donation of a wheelchair accessible van with a lift kit.
• Help for the caregiver.

The recipients(s) may receive one or all forms of assistance. The number of recipients will be based on need, nominations, and available funds.
